Over the years our team has supported participants from across the State and provided them transitional support to this region.
These participants have been from the family home, other providers, mental health units, long term hospital stay, correctional centres and homeless, but they all had a unique requirement of needing specialist complex needs support.
The support we provide is based upon the individuals needs and will fluctuate over time with their progress and regression.
